In connection to Chris Paton's post on NZ Shared Care pilot video, the National Shared Care Plan Programme (NSCP) was formed to initiate pilot projects which trial approaches to shared care to support long-term condition management. The healthAlliance, working with regional DHB and primary/community providers in the Northern Region is engaged by the National Health IT Board to manage the programme. The healthAlliance has contracted HSAG to implement its Collaborative Care Management System (CCMS) to support these pilot projects.

Comment by Chris Paton on April 3, 2012 at 11:45 Thanks for posting Mark. Coordinating care across multiple providers is a hot topic at the moment. Have you seen the work going on in the US with Accountable Care Organisations? This link explains more - http://www.healthcare.gov/news/factsheets/2011/03/accountablecare03....
Hi Chris. Thanks for the link.
I heard it all started when Obama signed into law the American Recovery and Reinvestment Act in 2009 which also highlighted computerisation of health records and the adoption of information technology in health care. Since then quality health care provision has become a commodity and the use of shared care management system has become more attractive. As what is happening in the health information technology market recently, numerous types of CDSS software have come which is very interesting! Cheers
